Argentina’s Transplant System Goes Onchain with IOTA (IOTA)

IOTA powers INCUCAI’s new live, verifiable onchain system for Argentina’s transplant network, enhancing trust and efficiency.





Argentina has launched a live, verifiable onchain system for its national organ transplant network, powered by IOTA (IOTA). The initiative, developed in partnership with Latin American decentralized identity firm Extrimian, integrates decentralized identity and cryptographic verification into the operations of INCUCAI, the country’s national organ donation and transplantation authority. This system went live on the IOTA Mainnet as of June 2026.

This deployment digitizes and streamlines the complex, life-critical processes underpinning Argentina’s national transplant program. INCUCAI and its 24 provincial procurement agencies now operate using verifiable digital identities anchored on IOTA’s distributed ledger. These identities form the foundation of a tamper-proof system for document signing and notarization, ensuring end-to-end trust and traceability without exposing sensitive personal data.

Why It Matters

Organ transplantation is a race against time, with donated organs remaining viable for only a few hours. Coordinating dozens of stakeholders—hospitals, provincial agencies, and the national coordinator—requires airtight processes. Even a minor error or unverifiable record can cost a life. By anchoring processes on IOTA’s decentralized network, INCUCAI can ensure that every authorization, medical record, and consent is trustworthy and immutable.

The system achieves this without storing patient-identifiable information onchain. Instead, cryptographic proofs (hashes) of documents and actions are anchored to the ledger. This approach protects sensitive data while enabling anyone to independently verify the integrity of the records.

The Technology Stack

Extrimian leveraged several key IOTA components to build the system:

  • IOTA Identity: Decentralized identifiers (DIDs) allow INCUCAI and provincial agencies to issue and manage verifiable credentials following open W3C standards.
  • IOTA Hierarchies: Establishes INCUCAI as the root authority within a national trust federation, enabling it to accredit, suspend, or restore agency credentials as needed.
  • IOTA Notarization: Provides immutable, timestamped proofs of document actions without placing document contents onchain.
  • IOTA Gas Station: Covers network fees, ensuring officials and users don’t need to handle tokens or worry about transaction costs.

This integration aligns with IOTA’s broader goals of providing scalable, feeless infrastructure for real-world applications. The technology’s neutrality and resilience make it a compelling choice for critical public services.

Market Context and IOTA’s Trajectory

IOTA, launched in 2015, has been positioning itself as infrastructure for machine-to-machine communication, IoT applications, and now, national-scale public services. The network operates on a Directed Acyclic Graph (DAG) architecture called the “Tangle,” enabling scalability and feeless transactions. Recent technical upgrades, including the April 2026 adoption of the Starfish consensus protocol, have improved its reliability and decentralized operation.

Despite these technical advancements, IOTA’s market performance has remained underwhelming. As of July 30, 2026, the token trades at $0.03933, down 1.06% over 24 hours, with a market cap of just $109 million. This reflects broader market volatility and ongoing skepticism about the project’s ability to translate technical milestones into sustained adoption and financial returns. However, real-world deployments like INCUCAI’s system could help validate IOTA’s utility in the long term.

What’s Next

Argentina’s transplant network is now set to expand the system’s use. Electronic signatures are rolling out across SINTRA, the National Procurement and Transplant Information System, starting with heart and lung transplant registrations. The goal is to digitize the entire workflow, eliminating paper-based processes and manual coordination steps.

For IOTA, this deployment serves as a showcase of its capabilities in delivering transparency, security, and efficiency at a national scale. With its open standards and privacy-preserving design, the project could serve as a blueprint for similar initiatives in healthcare and beyond.
